What can i do to help my mother?

My mother has COPD,she has Congested heart failure,High blood pressure,she is also a diabetic,she is also overweight, she has a sleeping disorder also, which she has a machiine for that but refuses to sleep with and she is a heavy smoker and she refuses to quit smoking.. so please anything in mind that i could do to try to help her please respond.. she is in the hosptial right now in ICU for these things so i am tryin to get some kind of help for help... and she is hard headed she doesn't want to listen to no one..so please give me something to go on..oh and she is 47years old about 5'7 weighs close to 400 if not over 400..thank you for your help.
